Referral Process
Evolve Psychiatric Services Your Partner in Mental Health
- Obtain a Referral Letter: Get a referral letter from your GP or primary healthcare provider.
- Send the Referral: GP clinics can send all relevant information, including referrals, via Healthlink ID: evolveps, Medical Objects, fax: 07 3812 9002, or email: admin@evolvepsychiatricservices.com.au
- Please note that email is not a secure method of communication. If you have any concerns, feel free to call us, and we can arrange an alternative, more secure way to communicate.
- Triage Review: Our triage team will review the referral letter within 5 working days and contact you to offer an appointment with our psychiatrist.
- Schedule Appointment: We will coordinate with you to arrange a suitable date and time for your appointment.
- Payment: Payment must be made in advance of the appointment.
- Receive Appointment Details: We will send you an email with a video link for your tele-psychiatry session and a text reminder the day before your appointment.
- Tele-psychiatry Session: During the session, you and your psychiatrist will discuss your mental health concerns.
- Post-Session Communication: Your referring GP will receive a letter from the psychiatrist with the recommendations made during your session.
- Discuss Recommendations: Consult with your GP to review and discuss the psychiatrist's recommendations.
